Bhagavad Gita: Chapter 15/Verse 4

ततः पदं तत्परिमार्गितव्यं
यस्मिन्गता न निवर्तन्ति भूयः ।
तमेव चाद्यं पुरुषं प्रपद्ये
यतः प्रवृत्तिः प्रसृता पुराणी ॥१५- ४॥

tataḥ padaṃ tatparimārgitavyaṃ
yasmingatā na nivartanti bhūyaḥ ।
tameva cādyaṃ puruṣaṃ prapadye
yataḥ pravṛttiḥ prasṛtā purāṇī ॥15- 4॥

... then that place is to be sought after, in which those gone do not return again. Indeed, I take refuge in that primal spirit, whence the primeval activity flowed.

eva (adverb) (indeclinable) = indeed  

ca (conjunction) (indeclinable) = and 

ādyam (masculine, accusative, singular) = primal 

puruṣam (masculine, accusative, singular) = spirit 

prapadye (present tense, 1st person, singular) = I take refuge  

yatas (adverb) (indeclinable) = whence 

pravṛttis (feminine, nominative, singular) = activity 

prasṛtā (feminine, nominative, singular) = flowed 

purāṇī (feminine, nominative, singular) = primeval
